Preparing for Your Pet’s Visit at Advanced Veterinary Care Center/Animal Hospital
Appointments
Appointments with our veterinary specialists are generally made by referral from your primary care veterinarian, although we may accommodate some appointments without a referral in selected cases.
Patient Information Form
Once you’ve scheduled an appointment, we request that new patients please take the time to print and complete a patient information form to bring with you so that we may create a patient profile here at AVCC. You may also email or fax the form prior to your arrival or you can bring it with you.
These forms are also available upon your arrival, however, filling them out ahead of your visit saves you time and allows you to easily interact with your pet without distractions.
Other Paperwork You Should Bring
To help assist you and your pet as quickly and thoroughly as possible, please also bring a copy of all previous veterinary paperwork or records to your first appointment, along with all available medical records, medications, dietary information, insurance documents, and anything else you believe is important.
Other information that is helpful to bring includes microchip information (for patients with existing microchip), and a record or listing of any current medications your pet may be taking.
